FILLING TIER-1 GENERAL EXTENTION FORM

Senior members of the forum, I need help with filling in a few details in the Tier-1 General extension form.

What do i have to fill for the following fields

B11 - Home office reference number
B12 - Worker reference number
B14 - Points based system migrant reference number (is it the visa reference number on the stamp? where do i find it?)

D3 - What date was the entry clearance or visa issued (is it the date of application or date from which the visa was valid)
D4 - Please enter the reference number on the entry clearance or visa which can be found in the applicant's passport (again is this the same as B14?)

B27 - Which occupation do i select in view of the fact dat i have the following under the jobs i do - Computer related services, Networking, Database , General Services (which includes supplies of items not IT related).

Am applying at the same time with my husband who is my dependant, what do i fill where it was stated in the dependant's form about the last time i as the main applicant did an application to the Home Office

PLEASE DO ASSIST!!!!!!

If you have MS word 2010, you will get wide varieties of invoice templates.
Use any one of them or just google it, you will find many links.
