EEA Visa transfert to new Passport

Hello,
Already a family member of an EEA National. wife is not working as we have a baby of 1 and half YO and prefer for her to stay home for now. Changed my name a while back but haven't bothered to change my passport, now time has come to sort everything out with the standard. new passport done and will submit a new EEA2 application.
I'm employed on a long term contract and will last until the end of the year maybe and I don't see any section on the EEA Application form about this case? I'm the one supporting my family and I don't know what and where to write this statement?

any one has an experience like this one?
