Tier 1 extension - Pvt Ltd company - Document info reqd

My friend has a private limited company and is the director of his company and he owns 100% share of it. He will be going for his Tier 1 extension in Nov, can someone help with what documents will be required for him to submit to UKBA while applying for an extension? Is there a requirement for the accountant to be registered with some body in the UK?

Any detailed help would be appreciated!!
