Tier One (General ) Visa Extension

Hi,

I am applying for Tier one (General) Visa extension for myself and my wife (dependent).

Please can someone advise which version of application forms need to be used for both main applicant and dependent.

The forms available for download on the UKBA site are always the current and correct version; but as opposed to what other source; where would you be able to find pre-printed versions of application forms that may have lapsed?
