Tier 1 (General) second extension - Immigration History

Hello all

I have tried to understand previous, similar posts on this topic, but still am not 100% certain how to answer this.
My first tier 1 visa + entry clearance application were made and granted from outside the UK (2007). My first extension was made (and granted) within the UK (2009). I am now doing my second extension from within the UK.
Question D1: Did the applicant obtain entry clearance or a visa before entering the UK? Yes / No
My only entry clearance was done initially, not for my extension. Is the answer here Yes or No? (If the answer is No, diregard D2 - D4)

Question D2: Where did the applicant obtain entry clearance before travelling to the UK?
Question D3: What date was the entry clearance vissa issued?
Question D4: Please enter the reference number on the entry clearance or visa which can be foudn in the applicant's passport or travel document.

Could someone please explain to me, in plain and simple English, how I should answer these questions?

This is simple

Entry clearance means permission to enter the UK. So entry clearance is always given when you are outside the UK which you got in 2007.
D1: Yes
D2: The place where you got initial visa in 2007 e.g. Beijing
D3: Date of your first visa in 2007
D4: Check reference on your first visa (2007 visa). This could be in your previous passport.

How certain are you?

I'm only asking because all this information would've been completed in my first extension in 2009. If I enter this same information for my second extension, there will be no record of my current visa number/office of application on my new extension form.
