Is there an HSMP Review form ?

Is there anything like an HSMP review form ? Few posts mention inclusion of the form alongwith your decision letter. Nothing was enlosed with my decision letter.

People who have had successful reviews, did you use a form or was it just a letter with clarifications etc

as hsmphopeful's posts, it is just a request by email or by fax to the HSMP team in Sheffield. In additon, put all your thoughts about how you caould make them convinced that you deserve the 50 points

to make sure it is better to send them an email to ask them what to do, inbetween you can organize your case

You need to send a written review request to them, either by fax or letter. You may include the information submitted before but highlighted this time to state the reason for request. Any additional information will be deemed as re-apply. They have all the photo-copy of documents you submitted before.

The address and fax number are on the decision letter they sent to you.

Of course it's best to get a reply from them for specific advices, as each case may have different requirements.
