Hi,

COS is valid for 3 months from the date of issue and you are allowed to apply for an extension up to 3 months before your current visa expires. So, issuing a COS in Jan for Apr extension sounds about right. I am due for extension on 14 Apr 2013, so my HR told me that they will issue one in mid Jan. Issuing one shouldn't take more than a day. They just have to enter your details into UKBA database and provide you a COS number.

It is quite safe to apply on your own for straight forward cases - when you have the awareness of double checking everything you need before applying. If you need your things sorted out quickly and don't trust posting your passport, then you may apply for in person appointment at one of the PEO's. I tried that before many times. In my experience Sheffield PEO is the best and Croydon the worst.

Postal processing time: 3 - 5 weeks.

However, you need not wait till Apr. You can apply 3 months before it expires.

In person:
same day decision in most cases. BRP in 5 working days.

For tier 2 general - apply from uk - see right hand side column for form and guidance:
http://www.ukba.homeoffice.gov.uk/visas ... /applying/

it says for dependants you have to fill separate PBS dependant form

You can always find a form using form finder:
https://apply.ukba.homeoffice.gov.uk/iapply.portal

PBD dependant for tier 2:
