hi & Happy new year to all.

I have ILR [Dec 12] so I applying for baby's British citizenship

regarding SECTION 5: REFEREES AND IDENTITY

Can one ask GP and health visitor to complete this ? Do they normally do it can someone please share experience ?

If I have to pay by cheque, to whom it should be payable to ?

Hi Keepsim

How old is the baby?

You can ask your gp or health visitor,They may sign it or refuse to sign as they may tell you they dont know you.

Look in the home office website the list of people who can sign or in FAQ in our british citizenship section.

The cheque is payable to The Accounting officer,Home Office
