Limited Company and salaried employment

I currently work through a limited company structure with 1st contact. I earn more than enough to qualify for my HSMP.
I need advice on the following.

I know I'm deemed an employee and shareholder active in the day to day running.

I can provide:

p60
original bank statements
Dividend vouchers
letter of employment stating my salary and dividend payouts.
However, I don't have any payslips. Can I substitute them with a statement of earning showing my salary?

Was just wondering what a dividend voucher is?
Also what do you mean by a dividend tax credit?

I as aswell receiving a part of my salary in dividends. All I get is a monthly payslip in a plain A4 paper without any company seal / signature. I know this cannot be used as a payslip.

I was told that I will be getting something like a annual dividend receipt. Is this the same as a dividend voucher?

Hi Lotter,

When you say you dont have any salary slips.. what do you mean?

Did 1st contract paid all your contract earnings as dividends and no salary... out of curiosity want to know why is that???
