Tier 1 dependant from Paakistan online form

I intend to apply for my dependants from Pakistan and have fue questions and would be thankful to anyone who have exact answers to these: Thanks everyone that have read my questions even if have no answers:

I am on Tier 1 General and got my 3 year extension right now:
I intend to apply for my wife and two kids (a son and a daughter) dependant visa from Pakistan through following link http://www.visa4uk.fco.gov.uk/ApplyNow.aspx

1- Do I need to make three seperate applications for wife, son and
daughter each?

2- In a question regarding main applicant passport they ask:
Place of Issue(In details they wrote CITY and country Name)
So I got my passport from Pakistan embassy in London so answer in
this case would be LONDON, UK ?
As in the next question they have asked Issuing authority, wihc will be
PAKISTAN
3- In "Main applicant employement details section" a question is: When
the job is expected to finish? If you dont give any answer you cant go
to next page and you cant write any text, so I dont have realy answer
to this because I dont have a contract end date? while u need to have
a date here then you can go to next page

You can put the date the visa of main applicant valid until.

3. I understand this is a bit tricky. If a date is really needed, just write down your current visa expiry date, because at the moment you have limited leave and cannot work beyond that.

Also get your Nikah Nama registered AND endorsed by the relevant Union Council or Cantonment Board of your area in Pakistan

My spouse is apllying from Pakistan as a dependant of Tier 1 (General) from Pakistan and need to fill online application form (VAF 10 almost similar to one in pdf form)

Please help if you have exact informations.Thanks

1- Two questions on the form are almost similar because of its same answer:
i- Have you ever travelled outside your country or residence, in the
last ten years?
ii- Have you ever travelled to the UK,in the last 10 years?

As my wife came to UK previously two times but never visited to any other country so is the answer yes to both above questions?

If Yes to both then it ask in two seperate question again

iii- You stated that you had travelled outside your country,
Please give the approximate date and duration and purpose of your
stay/s?
iv- You stated that you had travelled to the UK,
Please give the approximate date and duration and purpose of your
stay/s?
So now the information provided will be same in question (iii) and (iv), that is details of visit to UK?

Does this make sense or should we right NO to question (i) in which case question (iii) will not exist on form

If your wife has visited UK two times within last ten years and she has never been to any other country during last ten years, then the answers are

i Yes
ii Yes
iii Details of her both visits to the UK
iv Just type all details exactly same as iii

In the Online form of the dependant of Tier 1 (G) applying from outside of UK (from Pakistan) where should one give details of what supporting documents one is attaching to his application? As previously my family visa was refused due to lack of supporting documents. This time I have too many documnets (like Joint accounts/ telphone bills etc) to support but to avoid its missplacing or in case it detached from the application, that I have sent such and such documents.

I was thinking to reply, YES in response to a question :
Is there any other information you wish to be considered as part of your application?
and then to reply, 'Please consider "Appendix A", A hard copy attached to the print of this application form' and then give all details on two pages as Appendix A and attach it as mentioned.
