four boxes in passport application form

Hi All,

Section-4 of passport application form has four boxes to cross (just under "Parents' details" heading). What are they for? Do we have to tick any of those?

My wife was equally puzzled. I pointed out that the form says (not very clearly) that they should be left blank.

There is no information for the applicant to provide on those boxes.

I was told by IPS CW to leave them blank when I applied for my baby passport last saturday(fast track).

They should be left blank as outlined in the form (and the guide).
