Can you please advise whose name should be printed in the "Section O - Your Declaration" for under 18 PBS dependent.

Should it be name of parent who is signing or name of dependent whose application form it is?

This should not be of much worry. Just make sure whoever is signing, his/her name is clear.

Generally, name corresponds to person whose signatures are put on.

As far as I remember, I had put up applicant (minor) name, and then my signature. Also, added comments that signature belongs to me as applicant is minor.

There was no problem.
