Do we need Sponsorship undertaking form for PBS Dependent

Do we need to submit the below for the PBS dependent Visa?:

1. Sponsorship undertaking form. If yes is it SU 07/12? If it is different which is the exact form?

2. Letter of Support

Just curious, what is the document that will prove that Main applicant has a consent to the applicant applying for the PBS Dependent visa?
Are the various documents submitted of the Main Applicant are considered consent of the main applicant to the applicant's visa application?

I think some document with some sign of the Main applicant agreeing would be required and sponsorship form is just that.
