Tier 2 online Application

please help me on following question in my application form please?
It ask if I ever refuse visa/ entry clearance?
first time when I got visa it was in appeal so should I say Yes?

Take it your visa was refused and then given a chance to appeal which means 'Yes'

SO actually I should tick yes and give the detail in box that My Intial application was refused then successful in appeal?

I am filling paper form now as online application is not happening few question while going through form

1- While entering Post code for address some time one less box then actual postcode can I write out side box?

2- they ask for previous addresses but only provide to enter one address but I lived in 3 places during last five years what should I do?

3- Sponsor license number 9 characters but provided boxes are only 8?

4- Do I need to give any previous reference number in application other then my Biometric reference number?

I wrote outside the box so it's okay. I did not give any reference number.

and what you wrote in country of issue and issuing authority about passport
as my last passport was renewed in UK

Hi,
I am on Tier 1 visa and valid until 30th march 2013. Altough I can apply for an extension for Tier 1 I am not willing to do so.
Alternatively, my company is planning to apply a Tier 2 for me. If in case the application for Tier 2 is refused and this refusal informed to me after 30th March 2013, Am I still eligible to apply for my Tier 1 in April or sometime within UK.
Your help is much appreciated.

As I have said,,,you do not have to! I did not enter all of them as there were no space. If they want you to fill it they will tell you.
