Required Docs for 50k Tier 1 Enterprenuer

According to that thread, I list my docs for Tier1 Enterprenuer visa. I have the PSW now and already got 50K in my personal account. Could anyone help me to check if I omitted anything?

1. Passport
2. BRP
3. Applicaiton form
4. Photos
5. Personal bank statement showing £900 for 90 days
6. Personal account statement showing 50k
7. Business account statement showing money coming in from contracts
8. contracts (Trade Activity copies with each page signed by both team members)
9. Form Ct41g (copy) from HM Revenue & Customs
10.Advertising Material to show Business Activity
- business plan,
- business cards,
- website printouts showing names
11. Incorporation certificate
12. A Current Appointment Report from Companies House
13. Degree Certificate (Original)

Could you pls explain:
5. Can I save the 50K in my personal account not a business account?
10. Must I submit the business plan? Must the website printouts show my name rather than the company name?
11. How can I get the incorporation certificate? Is there a sample about this?

One more thing, according page 24 of the guidance: you must provide advertising material showing your name (and the name of the business if applicable)....

How should I understand this? Does it mean that I must supply my name and my business name together? Or, I only need to supply my name or my business name?
