Very urgent- tier 1 dependent form - change name

Hi all,
I am submitting application for extension tomorrow by post. I have only question in regard to change name :

Actually in last passport when first time when visa was granted under tier1:
My wife has used just fist name and my son as well. Their passport were only having first name. But now after we have added last name both my wife passport and my son's passport.
Process we followed: added surname after marriage for my wife passport

for my son : advertisement in newspaper and affidavit from notary

Can anyone please let us know whats sort of proof required for this?

we giving: marriage certificate and both old and new passports
do we need to make any cover letter?
any other proof required

This change happened recently, 8 month ago and all this was changed in London only

8 months is not recent .... :)

Your wife should send a change of circumstances form to UKBA within 3 months after getting new passport for updating their records... there is a fine of £1000 as stated in the website for non compliance...
In case you have old style vignette sticker on your passport, you may get away with the fine....

Do we need to do in this case?

There should not be any issue with name change of your wife and son. I was in the same situation where I had to change my wife's name from the one which was used initially for her tier 1 dependent visa. I had PEO appointment on 25th March and they didnt asked any supporting document for name change. I had submitted my wife's passport (with observation about name change). If you have the affidavit and other supporting documents, just include them as well. but there should not be any issues in your application due to name change. Good luck.

We have only old passport and new passport, marriage certificate and child's birth certificate.

affidavit and newspaper adv. were submitted for name change

Yes, we did fill the name change option as asked in the form. the diocuments u listed above is more than enough.
