Page 4: i am on hsmp+tier1. Should I say 'no' or leave it blank for the first question?

If I don't remember the finger print date, can i give the month & year?

1. If you're covered by HSMP Judicial Review then say yes otherwise no. When did you apply for your initial HSMP Letter?

2. Yes

Just that you know, a new version of the application form will be out after 6th of April
