Residency proof is your passport (new and expired) and you need to have been physically present in the UK exactly 5 years to the day on the date that UKBA receives your application.

No, there is no need for letters. See below link, page 14 of the AN GuideI thought you have to submit letters in addition to passports?
You can explain on the additional information page of the form that you have lost the certificate. Do you have a copy or any details as if you can provide this it will be easier.I wasn't sure what they mean by a 'confirmation' and thought the certificate is my only source to confirm.

Life in the UK: I re-checked the guide and it states 'your confirmation' which I am assuming doesnt need a certificate. - I used my test certificate for ILR and when I applied for BC, they took the test certificate for submission again. As I have said, you can write in the Additional Information section of the form that you have lost it but have used it for ILR.
Another trivial question: 1.1 on the form (UK border agency reference) where can I find this? you can leave this blank, it is not necessary to fill it in.
