Which form next?

Hi,

After a successful HSMP application what is the correct Entry Visa form to be filled in in order to live and work in the UK? Application will be made from Russia.
Have looked at VAF1 but am worried as it doesn't seem to mention HSMP in it.

Could someone please point in the right direction for the correct forms/procedure/and supporting documents required. On the official website it says only the HSMP approval, photos and passport are needed, is this enough?

its VAF1 ... you can't use any other form...if you're outside UK.

just tick 'Other@ and specify HSMP on the type of visa applied for

What supporting documents need to submitted along with the form though?
Is it really just the HSMP approval document, 2 photos, passport and nothing else?

In the HSMP notes it says a letter of intended stay is needed, any examples of this letter?
Is an interview required as well and if so how in-depth is this and will any documents be needed to take along?
