Previous earnings - what exactly is included in income ?

Previous earnings - what exactly is included in income ?

Are health insurance and pension payments also counted in total earnings for HSMP ?
Or are previous earnings only the amount of money I get in cash plus the taxes without health insurance that my employer has payed for me ?

Payments without health insurance and pensions!

and the HO considers the GROSS ANNUAL INCOME (BEFORE TAX)
