New born baby

Hi

I would like to get a visa for my new born baby in the UK. I am on tier 1 general.

1) Is below the correct form that I need to fill in?

2) how much maintenance requirement I need to show? I have only one child and my wife

3) Do we need to send mine and my wifes passport or just the baby's?

4) I understand they will also ask my statements and full UK birth certificate.

Anything else I should know?
