What do you mean by a sample ...

You need to ask your bank to send you the bank statement for the last 3 months. This can be used to prove that you can support your dependants if you have sufficient funds in your account.

Alternatively, you can ask your bank to print out the statement for the last three months and stamp every page and provide a covering letter.
