Person Identity Number ?

Hello . Just have a quick question . What should i write here ? from where i can get my personal Identity number as i am going to apply first time for Citizenship after 5 years . My husband already got Irish citizenship this year .

dept number is the one start with 69.if you have the new one then put the new one other wise put dept number on it
