Urgent: Which form to use for ILR application?

I have been under the impression that the right form to use to apply for ILR via the long residence route is the LR Form.

But in the forum, I've noticed that Set O form has been used to apply for long residence too.

Can anyone clarify which is the right form to use to apply for ILR via the long residence (10 years or more) route please?

It used to be SET(O) before 6th April 2013, but the current form is SET (LR) for long residence.
