For my own understanding how come you submited an application for EEA4 just after 3 years of receiving EEA 2 :?: Are you not suppose to do so only after you complete 5 years of residency from the date you receive your EEA2?
GOD BLESS!!!

For the year 2008-2009,self assessment is not enough.you are require to sent invoices,or bank statements which shows self employment activities.
For the year 2009-2013,you are require to sent p60s and at least 6 month payslips each year.
All employment letters are vital in your case.
Rest of the documents are fine.

Best of luck.

It's better if you sent it again and avoid any delays.

They are required in law to take up to six months to deal with your application. Therefore there is nothing illegal about 4 months.
