EC application for self and wife as dependant

Hi all,

I am planning to apply for EC for self and same time i want to apply for my wife as dependant.

Can you please suggest the right method of filling up the form or the correct procedure. ?

It is possible to apply and take wife together as dependant right ?

Any other special requirement or something which i should take care....?
