Switching from tier 4 to tier 2 .? Documents?

Hello everyone,
I am switching my visa category from student tier 4 to tier 2,

What document should i send with my application.?

And my employers has to advertise the job or not?

Because i use to work with them in 2010 and then i came London for my studies. Now my education is completed.

Any suggestion must be appreciated.
