Lost Indian Passport & OCI

Dear All,

Today while going out for a photo copying indian passport, my wife lost her Indian passport on the way home.!

I am planning to apply for my wife's OCI but not sure what to do?

Can someone please guide on how do i now get the passport surrender certificate as the passport is now lost !

I went to police station but they did not register the complaint and provided me a number ; to put in my application incase HCL london wants to get in touch they can confirm the fact!

I reviewed some topics / threads and noted i need to submit some affidavit, but not sure what to write in that certificate to get it notarised!

If someone can provide me a draft or further guidance on this would be appreciated!
