Applicant name is your son so it has to be his details.
You can mention your details under card holder name if you are the card holder

One payment slip will do.
Other documents will be fine.
I suggest going through NCS as you get your original documents back and they check your application before submitting to UKBA.

What date was your child born in 2000? The rules have changed in October so it will affect your case.

Normally, you don't need to fill in section 3.

In that case, your child is probably already British and can apply directly for a passport. No need to register him using MN1. The rules at the time of your child birth were that if you were exercising treaty rights at the time of the birth, the child is British. (the rules today require PR). Were you (or the child other parent if he is a EEA national as well), exercising treaty rights at the time of the child birth? Just provide that proof to the Passport Office. See the passport office guidance (page 3).

The rules changed in October 2000. Your 2001 born son will need to register to become British using form MN1. His PR is not relevant to the application. What is important to prove is that one of the parents obtained PR (sometime after his birth).

1.3 - 19/08/04 is fine.
3.13-3.15 - this action is not required as you have proof of PR (sticker in passport).

A for documents, one parent documents is enough (better to send mother evidence). So 1,2,7,8 should be enough.
