Criminal convictions - Road Traffic Offence ???

Hi All
In May 2004 I had a car accident at work a few months later I had a hearing in the court and I been convicted of Driving without due care and attention / Dangerous driving.I do not remember exactly what was the charge and exactly date.(October/November 2004)
I lost all paperwork.I remember that I received a £ 450 fine and 9 points on my Polish driving license.
From 2006 I have a full UK driving licence and it's clear without any points.
2 years ago I had a Criminal record check and it was clear as well.
I'm wondering if I need to give information about the case in Section 3: Good Character Requirement point 3.6 ???
If YES, what should I write?

Then in 3.7
Country Where Convicted UNITED KINGDOM
Nature of Offence: Traffic offence-Driving without care and attention
Sentence Given: 9 points and GBP450 Fine
Date Sentenced Put 31st Oct 2004

This should have no adverse impact on your application.
