Filling up the Payment Slip

Hi friends,

Need some urgent help. Now I am using my friend's credit card to pay for HSMP. I am filling up the Payment Slip.
So for questions 1 to 8 I plan to give my details like name address surname blah blah...
But for questions 11 and 14 I believe I have to put my Freind's name and his signature. My friend is the one who is paying through his credit card.

Please correct me if I am wrong. This sounds silly but I just wanted to cross check.

I think the application form is quite clear
it says applicant details, that is you
it says contact anem for payment queries
that is your friend as he/she is paying
then it says method of payment
which is your friends credit card
