please help for filling british passport

hi, i am apply for my son passport and on the section 4 it say mother's full name(surname first)

my maiden name is : laa

we are maried and follow to husband surname : mrs man


just want to know i write laa or man or both surname


and also on the section 5 certificat of registration it say place of issue what i write home office liverpool or home office london
