Tier-1 Extn form New Q C6

HI,

My wife is applying for Tier-1 extn and would like to know expert's advise on the following question:

C6. Do you have any passport or travel document which contains a previous grant of leave or which you used when you last travelled to the UK?
Yes document enclosed, passport/travel document number is
Yes document not enclosed, it is: Lost Stolen Expired
No

This is her second extension. She entered UK on individual Tier-1, and date of entry was 6months after the Visa approval date. This visa was stamped in her passport.
The second extension was in 2012 and this time she got BRP card.

Last time we had traveled to India and came back in Jan 2013 and we used BRP card for entry.
