SET (O) form query

I have query regarding, do I need to fill just one SET (O) application for myself and 2 dependents (or there will be 3 different forms)?

I am on tier 1 (general 3 +2) category and my dependents are with me in the UK since last 3.5 years.

Just one . Include your dependents in dependent section of SET(O).

same form if the dependents are your spouse and children under 18 yrs.

Guys just noticed they have released new version of SET (O) form which is 12/2013

I have only one week left for PEO appoinment.

Should I now fill this new form SET (O) 12/2013?

I am on tier 1 (general) visa.
