can a tier 4 dependent extend his/her stay separate from the main applicant.?

if yes which form will be used for this?

NO!
As it say, its "dependant" on main applicant. If he/she wishes to switch into another category, then leave the country & apply from the home country.

Hi

He is not switching category but extending his stay as dependent of tier 4 student separately. his wife visa was already extended. he is applying for extension on behalf of his wife separately.

Why didn't he apply with his wife at the same time? IMO, technically he has got no leave at the moment because his previous visa or leave to remain would have been exhausted when the main applicant's (wife) leave was extended. Anyhow if he is still under the 28 days period, and he meets all the requirements, then he may apply.

His leave is till valid for five months.

How? When the wife applied for extension & she got new leave to remain & at that point of time, her existing leave (visa) was replaced by a new one (current), IMO, it also meant that the husband's leave (visa) ceased to exist because he did not apply at the same time. You can wait for any other member to confirm this.
