ILR from Tier 1 (General) - English, Life in UK Test

Just want to share to help anyone applying for ILR based on Tier 1 (general) after 5 years. I successfully applied for ILR visa in Dec 2013.
I was on Tier 1(General) visa for 5 years.

You need the following documents

(1) proof of earning documents e.g payslips, bank statements
(2) Life in UK test for main applicant and dependents documents
(3) English proof for dependents only (NO proof is required for the main applicant)

NOTE: You can apply 28 days before 5 years.
NO proof is required for the main applicant
No document is required for absences if these are less than 180 days.
