Settlement dependant sign on SET O

Hi,

I would like to apply ILR for myself, spouse/dependant ILR and child ILR.

1. Can I use one SET O application for my ILR and dependants ?

2. If yes, in which section of SET O form spouse need to sign the application

3. 2 years spouse continues stay start is not specified in form.
Where do I need to fill on the SET O form.
