Naturalisation Language Exemption!

Hi,
Is it possible for anyone to post link for Nationality Case working instructions especially in regards to medical exemptions for Old aged individuals please. I am unable to find it on UKBA web site..

Also what are your thoughts if an old age lady applying for naturalisation, she is given medical certificate from her doctor requesting UKBA for her to be expemted for this test. Would it suffice?
