ILR SET (O) questions for PBS Dependant

Hi,

I am planning to apply for my wife ILR visa next month using SET (O) as she is on Tier1 (General) PBS dependant and have few questions.

Visa granted: 4-Jan-2012
First entered in UK: 2-Mar-2012
Extension as PBS Dependant: 1-Nov-2013

1) What should I select in "section 3 - which category"? Tier1 (General) excluding HSMP or PBS Dependant?

2) In the "section 5 - home and finances", she is working part-time, do I need to mention my income in 5.3 to cover up monthly rent expenses?

3) Do I need to sign in the "section 12 - Consent for the Home Office to Request Verification Checks?" As my finances (payslips & bank statement) will be used to cover up maintenance, rent and other expenses.

Please advise, many thanks in advance.
