Tier 2 to ILR - Driving Ban

Hi

Looking for some help with this one, I am in the uk on a tier 2 sponsored work permit visa which runs out in March 2014. I received a 14 day driving ban for speeding in September 2011. This was a court appearance, do I need to include this under section 7 of form SET (O)?

Also I can't remember the exact date, only had to pay court fees and didn't receive any points on my licence, can I just put September 2011 on the form? I also don't know what code it was, does this matter?
