Tier 2 extension application form

I am a bit confuse, I have found the application forms - form T2(W), but it says that to use this form for Tier 2 extension;

1. you must have previously been given permission to stay for less than 2 years as:
- Business and Commercial work permit holder (which includes ICT work permits);
-Sports and Entertainment work permit holder;
-Jewish agency employee;
-member of the operational ground staff of an overseas-owned airline; or
-minister of religion, missionary or member of a religious order.

Additionally, you must meet the following requirements:
2. you applied for, and were granted, leave to remain under Tier 2 through the transitional arrangements in place for migrants in these categories;
3. you are still working for the employer who assigned your certificate of sponsorship, and being paid the appropriate salary for your job;
4. you intend to continue working for the same employer;
5. you currently have valid leave to remain in a Tier 2 category (General, Intra company transfer, Minister of religion or Sportsperson);
6. you were granted the maximum period of 3 years' leave to remain under Tier 2; and
you can prove that you still meet the conditions of your existing leave.

My question was I am qualify for items 2 to 6, but not 1. Can I still use T2(W) form? If not, then which form should I use?

Hi,

I am about to submit my Tier 2 Extension application. I was wondering if I also need to submit 'Points based system form' together with the Tier 2 Extension application?

If I do need to submit 'Points based system form' with my visa extension application, which form is the correct one? I found 2 forms (links below);

1. Points based system form (appendix 5)

I noticed both forms stated 'This form is for use outside the UK only.', but I am currently inside the UK under Tier 2 visa.
