Switzerland tourist visa from UK

Hello ,

I am on Tier 1 Visa in UK , I am planning to visit Switzerland I have appointment with swiss VFS for schengen tourist visa . Can anyone suggest which all document i need to carry ,
I understand I need
1) medical insurance
2)travel ticket
3)hotel booking
4)passport , UK visa
5)address proof
6 ) I am confused do I need to carry uk bank statement , employer letter , payslips please advise

I applied for Schengen via TLS contact and advised to bring following:
