Change of circumstances - Name change

Hi All

My wife changed her name post marriage and we understand that we need to report this to UKBA using migrant change of circumstances form. She got the new passport issued from Indian High Commission reflecting her new Name. We've got couple questions if someone would be kind enough to answer those

1) Does she need to send her old and new passports along with the "Migrant change of circumstances form"?
2) Does she need to send her BRP as well along with the form?
3) Is there any fees payable to UKBA with this form?
4) How long usually it takes for UKBA to do these kinds of amendments and issue new BRP?
