How to calculate the previous 12 months & how to pay ?

Hi ,
I want to send my HSMP form now. Which month to which month pay slips I need to attach ?
Will Apr-2006 to Mar 2007 sufficient ? or May 2006 to Apr 2007 OK.

Also I want to pay by UK cheque. ( A UK friend is giving his cheque)
In first page of the form , what details I need to write ? Does my UK ( as the account holder )has to sign in the first page?

I believe the first page is about the person who pay for the application not the applicant. But may be wrong, that is my interpretation of it.

The 'Contact Details' section in the 'Payment Slip' page should have ur UK friend's details. I got it confirmed from HSMP team.
