Yes. Page number 8. The month of Date of Birth is on right most of the page :)

Dont Worry Chaps, just print the form as it appears and that should be fine. I have been through the application using the form having format issue and was not asked any question on it. Leave the payment page Blank if you have booked appointment at PEO. They will consider the print out of Payment sheet.

The alignment issues are resolved. Double sides issue is still there though.

Hi,

I am still not able to print the new form properly. The boxes for entering details like name etc. are are not completely printed. Last box appear to be truncated.

Can someone help?

Does anyone have a soft copy of Set (O) form for 2013 ? I applied it Sep 2013 - unfortunately i dont have a soft copy.

I have got the ILR; however, I do need to show the old form to my organisation.

Any help is greatly appreciated.
