I want to apply through NCS , I will apply with my passport application, do i have to pay passport fee separately?

Passport fee sheet is the last page in the guidance booklet you get with the form, they come together in one envelope.

1. AN Form
2. Passport
3. ILR BRP
4. Life in the UK Pass certificate
5. Trinity College B1 Certificate for English Language
6. Passport size pictures (I have 5) , not sure how many required both for passport and AN form

1. Letter from Employer confirming my employment
2. Bank statements
3. Accommodation proof (any letters)
4. UK driving license or photocopy of driving license
5. extra envelop to return my documents

I believe it is three, but not 100% sure as the joint service wasn't available when I applied in 2012.

You can but not needed. Handy to have I suppose just in case.

They won't be returned to you as you won't need them again. Suggest keep copies for yourself.

but he didn't put his passport number, is that compulsory, i can request him to put the passport number but i am not sure if he is comfortable to put the passport number.

I have booked an appointment with council, i have checked few old threads where few members were advising only Director of ltd company which is VAT reg too can be a referee. so that's why i want to confirm.

No mention of VAT registered until this one, which likely refers to LLP or Plc companies:
