Application for ILR as Tier 2 Migrant

I have booked for appointment for PEO application on the 24th of May 2012 as SET (O), my condition is summarized below:

My contract started 15/01/2007 and I was on student visa at that time, Thus I worked for 20 hours/ week till 22/05/2007. then I have left the UK and re-entered after issuing the work permit visa for me on 10/06/2007.
Date of Entry to UK as work permit visa was 18/06/2007,
I had an extension for my work permit & work permit visa till 15/01/2010.
My contract extended again and I have got Tier2 (general) till 28/01/2012
Last extension my contract on the same Job of was till 30/01/2013, I have applied by post, did biometric and still waiting for the visa.

1-My understanding is that legal for me to apply less than 5 years (within the 28 days) or I have to book after the 18 of June 2012. I will joint my wife and 3 children on the application?

2- Do I need as a Tier 2 and Work permit before the P60 for each of the 5 years? I miss the 2008 one, but I have the rest.

3-Do I need employment history from HMRC ? and I contacted them they asked for written request? Or I should ask it from my employer

4- Is it required for me as tier 2 migrant “an absences from UK letter” from my Employer? As I did not spend more than 90 days abroad per year or 180 in all 5 years? Only 132 days I did in 5 years.

Please help me as I am worried about the documents support and the 57 days holiday I spent in on year? I appreciate your experience and advise
