BPR (RC) - Name Change Application - Help Required

Hello Members,
I am applying for a Change of Surname in my passport. I have made the required changes in my new passport. Do i need to send a deed poll as evidence or is it enough to have you new passport with the required name.Eagerly waiting for your reply. Thanks
