TIER 2 ICT- Baby Born In UK

I was reading through this forum and looks very interesting...But couldn't find anything similar
Here is my Problem
Currently I am in UK under T2-ICT (Long Term) Valid up to Jan-2013.
Recently we had blessed with twin baby boys here in UK.I got Indian passport for them. My employer A-Rated Sponsor) had agreed to extend the certificate of maintenance to my kids as well.

My question is 1) Which form I have to use? I read somewhere to use FLR(O) form. Could you please confirm
2) What are the documents to be submitted?
Passport Of Kids
Salary & Maintenance Certificate( Which my employer had agreed to give)
Birth certificate of kids....Is there anything else..Couldn't find any proper guidelines in UKBA..Could you please help

I read somewhere to use FLR(O) form.

Not if you are a Tier 2 migrant.
