Immigration History question part D

I'm filling in Part D (Immigration History) of the extension application form, and have 2 questions:

1. What's the date of issue for my initial entry clearance? My Entry Clearance's "valid from" date is 08 Aug 2008. I know I received it in late June, and my biometrics appointment was 19 June 2008, but don't recall the exact date it was issued.

2. What's the "reference number"? Is it the red number in the upper right corner, or is it the VAF number?

If you don't know the exact date, mention 08 Aug 2008

What is the exact question / question number?

It's question D4 on page 20. My entry clearance looks like this:

I hope that's not your Visa copy. If it is, just remove it.

Answer to D4 is: The number in red on the top right corner.
